Amends Public Health Law to allow adoptees, birth parents, and biological siblings to pre-authorize the release of identifying information through the adoption registry using an appropriate form, rather than waiting until a match occurs

Enables the Department of Health to release identifying information (names and addresses) to consenting registrants upon receipt of final consent or pre-authorization

Requires the Commissioner of Health to promulgate rules and regulations for creating a pre-authorization form for use by adoptees, birth parents, and biological siblings

Takes effect 30 days after becoming law, with immediate authorization for the department to develop necessary rules and regulations before the effective date
