Mandates the pamphlet include information about required pre-surgery cancer screenings for conditions including uterine myoma, endometrial stromal sarcoma, and leiomyosarcoma

Pamphlet must be written in layperson's language and made available in English plus the six most common non-English languages spoken in New York, posted on the department's website as a printable file

Practitioners may provide the pamphlet to patients prior to performing the surgery, and the pamphlet must include contact information for reporting misconduct to the Office of Professional Medical Conduct
