Requires the Commissioner of Veterans' Services to identify all current or former military installations in New York where service members, veterans, or their families may have been exposed to hazardous substances, including sites registered under state brownfield cleanup programs or federal Superfund law

Establishes a voluntary registry for service members, veterans, and their family members to self-report potential hazardous substance exposure at military installations, including personal contact information, installation location, time period of presence, and any related medical conditions or deaths

Creates a public information program to educate affected individuals about the state registry, federal VA health registries (Agent Orange, Burn Pit, Gulf War, Ionizing Radiation, Depleted Uranium, and Toxic Embedded Fragment), presumptive conditions, and the disability compensation claims process

Requires aggregate registry data to be published on the department's website and, subject to appropriations, mandates a study analyzing reported medical conditions and their potential association with hazardous substance exposure

Directs the commissioner to notify affected individuals of exposure risks and advocate at the federal level for recognition of new presumptions of exposure and service-connected disabilities

Takes effect 90 days after becoming law, with all personal registry information protected as confidential under state public officers law
