Mandates health insurance coverage for persons with heterogeneously dense or extremely dense breasts to receive either an annual mammogram combined with breast ultrasound, or breast tomosynthesis (3D mammography)

Applies to individual policies, group/blanket policies, and coverage provided by medical expense indemnity corporations, hospital service corporations, and health service corporations

Coverage for these enhanced screening options cannot be subject to annual deductibles or coinsurance

Takes effect 60 days after becoming law and applies to all policies and contracts issued, renewed, modified, altered, or amended on or after that date

Amends multiple sections of the New York Insurance Law (sections 3216, 3221, and 4303) to ensure consistent coverage requirements across different types of health insurance policies
