Prohibits discrimination against home care patients based on sexual orientation, gender identity, gender expression, and HIV status, establishing comprehensive patient rights including the right to gender affirming health care, HIV-related treatment, and use of preferred names and pronouns

Requires all employees and contractors of home care services agencies and certified home health agencies to complete anti-discrimination training within six months of hire, with 3 hours required for direct contact staff and 2 hours for non-direct contact staff every two years

Mandates patients receive written nondiscrimination notices signed and placed in their records, along with information about how to file complaints with the Department of Health if rights are violated

Grants patients the right to sue for violations of these protections and voids any arbitration clauses related to these rights, while prohibiting agencies from requiring patients to waive these rights as a condition of service

Requires home care agencies with 25 or more employees to designate a chief diversity officer and report this to the department, with the act taking effect 90 days after becoming law
