Requires the Commissioner of Health to ensure tax forms include space for applicants to register or decline, with clear language asking "Would you like to be added to the Donate Life Registry?"

Takes effect 180 days after becoming law, with the Commissioner authorized to begin implementation immediately

Applies to individuals aged 16 and older, consistent with existing Donate Life Registry age requirements
