Expands existing insurance law requiring coverage of enteral formulas to also include infant and baby formulas for home use when medically necessary and prescribed by a licensed health care provider

Requires coverage for infant and baby formulas proven effective for treating conditions including inherited metabolic diseases, Crohn's Disease, gastroesophageal reflux, gastrointestinal motility disorders, severe food allergies, eosinophilic disorders, and impaired nutrient absorption

Mandates minimum coverage of $3,000 per insured individual per calendar year or any continuous 12-month period for infant and baby formulas

Applies to individual health insurance policies, group health insurance policies, and health maintenance organization contracts that provide prescription drug coverage

Takes effect January 1 following enactment and applies to all policies issued, renewed, modified, altered, or amended after that date
