Establishes the New York Healthy Incentive Program (NYHIP) to provide earned dollar rewards for SNAP recipients who purchase locally grown healthy foods at farm-to-consumer entities like farmers markets, co-ops, and community-supported agriculture programs

Requires the Office of Temporary and Disability Assistance to create a mobile application and website with an interactive map showing all participating farm-to-consumer locations that accept SNAP and NYHIP benefits

Directs the state to seek federal approval to automate SNAP benefit cards for community-supported agriculture subscriptions with automatic weekly deductions, and to add NYHIP incentives directly to SNAP cards

Creates a grant program to help farmers and farmers markets obtain technology needed to accept SNAP payments and participate in NYHIP

Mandates a statewide outreach program including informational leaflets in public buildings, public service announcements, and translated materials to inform eligible households about NYHIP benefits
