Expands the hospital-home care-physician collaboration program to require emergency medical services (EMS) as mandatory partners in community paramedicine initiatives, alongside hospitals, home care agencies, and physicians

Defines "emergency medical services" to include ambulance services and advanced life support first response services staffed by emergency medical technicians (EMTs) who can provide services beyond initial emergency care and transportation

Authorizes community paramedicine models to provide collaborative services to at-risk individuals in community settings to prevent emergencies, avoidable emergency room visits, unnecessary transport, and potentially avoidable hospital admissions and readmissions

Requires community paramedicine services for individuals with behavioral health conditions or developmental/intellectual disabilities to include collaboration with providers licensed under the mental hygiene law

Permits EMS personnel (EMTs and advanced EMTs) working for certified ambulance services to participate in community paramedicine programs, expanding their role beyond traditional emergency response
