Expands the definition of "infertility" beyond the traditional 12-month (or 6-month for women 35+) failure to conceive to include a person's inability to reproduce as a single individual or with their partner without medical intervention, and allows physician findings based on medical, sexual, or reproductive history to establish infertility

Changes IVF coverage from three "cycles" to three completed oocyte retrievals with unlimited embryo transfers from fresh or frozen oocytes/embryos from those covered retrievals

Requires IVF procedures to follow American Society for Reproductive Medicine guidelines, including single embryo transfer (SET) when medically recommended and appropriate

Prohibits insurance coverage denial based on a covered individual's participation in fertility services provided by or to a third party (such as surrogacy or egg/sperm donation arrangements)

Applies to large group insurance policies and contracts that provide medical, major medical, or similar comprehensive-type coverage in New York
