Authorizes New York residents with a child, spouse, parent, sibling, or grandchild actively serving in the U.S. armed forces to obtain distinctive "Blue Star Mother" or "Blue Star Family" license plates

Requires an additional annual service charge of $25 beyond the regular registration fee for these specialty plates

Establishes the "Blue Star Mothers and Families Support Fund" to receive all revenue from the plate service charges, with funds administered by the Division of Veterans' Services

Fund monies must be used exclusively for programs and organizations supporting parents and family members of active duty military personnel

Plates will only be produced upon receipt of either a $6,000 surety bond or at least 200 pre-orders, with the act taking effect 180 days after becoming law
