Authorizes Janice Middlebrooks to file a retirement option election form on behalf of her deceased husband William James Middlebrooks, who worked at Albion Correctional Facility and died on December 3, 2006 before filing his retirement paperwork

Allows the widow to select a joint allowance full option for disability retirement benefits, with the retirement deemed effective November 1, 2004 (when he was advised he could not return to work due to cancer treatment)

Requires the request to be submitted to the state comptroller on or before December 31, 2025

Retirement benefits will be paid to Janice Middlebrooks in addition to any other death benefits already received

Carries an estimated immediate past service cost of approximately $472,000 as of February 1, 2026, to be shared by New York State and local participating employers in the retirement system
