Establishes the "statewide initiative of not-for-profit guardians" within the Office for the Aging to provide guardian services for individuals over 18 who are eligible under Mental Hygiene Law Article 81 when no suitable family member, friend, or attorney can be found

Creates a free public helpline to provide guidance, resources, and referrals for individuals navigating guardianship proceedings or seeking alternative services

Requires participating organizations to be 501(c)(3) tax-exempt entities or not-for-profits in good standing with the Attorney General's charities bureau, with experience or demonstrated capacity in guardianship services

Directs the Office for the Aging director to select a lead not-for-profit agency to administer the program under a 5-year renewable contract, with responsibility for reviewing plans and awarding grants statewide

Establishes the "statewide initiative of not-for-profit guardians fund" in the State Finance Law and mandates annual reporting on guardianship cases to the governor and legislative leaders beginning January 1, 2027
