Expands the definition of "infertility" beyond traditional 12-month or 6-month timelines to include single individuals or partners unable to reproduce without medical intervention, and cases identified through a physician's evaluation of medical, sexual, or reproductive history

Changes IVF coverage from "three cycles" to three completed oocyte retrievals with unlimited embryo transfers from fresh or frozen oocytes/embryos from covered retrievals

Requires IVF coverage to follow American Society for Reproductive Medicine guidelines, including single embryo transfer (SET) when medically appropriate

Prohibits denial of coverage based on a person's participation in fertility services provided by or to a third party (such as surrogacy or donor arrangements)

Applies to large group health insurance policies and contracts in New York State
