Mandates that consumers be informed quarterly about available facilitation services and allowed to choose their own facilitator, with protections against involuntary reassignment during eligibility changes

Requires the statewide fiscal intermediary to maintain a publicly available, geographically organized listing of all subcontracted facilitators on its website, including designations for independent living centers and cultural/linguistic competencies

Prohibits the statewide fiscal intermediary from performing subcontracted services itself or creating subsidiaries to provide facilitation services, and imposes financial penalties for non-compliance with subcontract obligations
