Closures require a health equity impact assessment and proposed closure plan reviewed by the Public Health and Health Planning Council, with the commissioner making a final approval decision within 30 days of the council's recommendation

Public community forums must be held 150 days before a full hospital closure or 90 days before a unit closure, with 14 days advance notice and both virtual and in-person accessibility

"Reduction" of services triggering these requirements includes more than 15% capacity reduction within 12 months, 25% or more within 24 months, or 35% or more within 36 months

Annual reports to the legislature beginning January 1, 2027 must identify hospitals providing closure notices, proposed dates, and affected services
