Establishes a Patient Centered Medical Home Education Pilot Project to advance medical education in primary care through selected physician and advanced practice nurse practices affiliated with Ohio medical and nursing schools, with up to 40 physician practices and at least 4 advanced practice nurse practices selected for participation.

Creates a Patient Centered Medical Home Education Advisory Group composed of representatives from Ohio's four medical schools, osteopathic school, and five nursing schools, plus representatives from medical and nursing professional organizations, to implement the pilot project and develop curricula for training primary care providers in the patient-centered medical home model of care.

Authorizes development of primary care scholarship components within the Choose Ohio First Scholarship Program for up to 50 medical students annually and up to 30 advanced practice nursing students annually, requiring recipients to commit to three years of primary care practice in Ohio and accept Medicaid recipients as patients.

Extends the moratorium on most favored nation clauses in health care contracts with hospitals from two years to three years after the effective date of Sub. H.B. 125.

Revises Medicaid nursing facility tax cost reimbursement rates by allowing facilities with real estate tax credits in calendar year 2003 to use calendar year 2004 tax costs for determining rates for the period beginning July 1, 2010, and modifies fiscal year 2011 rate calculations to include an additional adjustment for nursing facilities with such credits.
