Creates a new Medical Orders for Life-Sustaining Treatment (MOLST) form to be prescribed by the Director of Health that documents physician or advanced practice nurse orders regarding hospitalization, life-sustaining treatment, comfort care, and CPR administration.

Establishes procedures for completing and managing MOLST forms, including requirements for patient or surrogate decision-maker participation, discussion of treatment preferences, and rules for amendment and revocation.

Specifies that MOLST form instructions supersede inconsistent instructions in do-not-resuscitate orders, declarations, and durable powers of attorney for health care, except when those documents were executed after the MOLST form and the physician is made aware of them.

Provides immunity from criminal prosecution, civil liability, and professional discipline for health care professionals and facilities acting in accordance with valid MOLST forms or CPR withholding/withdrawal provisions.

Creates a Medical Orders for Life-Sustaining Treatment Advisory Council with 31 members representing health care organizations, emergency services, advocacy groups, and state agencies to advise the Director of Health on implementing MOLST regulations.
