Requires written informed consent of the biological father before performing an abortion on a pregnant woman, whether the fetus is viable or not.

Pregnant woman must provide the father's identity in writing to the person performing the abortion; woman cannot knowingly provide a false identity.

If a woman identifies two or more possible fathers, the abortion provider must conduct a paternity test to determine the biological father before accepting consent or performing the abortion.

Exempts the requirement if the woman provides police reports or court documents indicating rape or incest, if the abortion is medically necessary to preserve the woman's life or health, or if the father is deceased.

Violations constitute "abortion fraud," a first-degree misdemeanor, or a fifth-degree felony if the person has a prior conviction; all consent documents and paternity test results are confidential and restricted to specific parties.
