Creates the Ohio Official Prescription Program administered by the State Board of Pharmacy beginning July 1, 2010, requiring all written prescriptions to be issued on official Ohio prescription forms with security features to prevent copying, erasure, and counterfeiting.

Requires prescribers to pay a computer maintenance fee of $250 annually (or as established by board rules) on June 30th to receive official prescription forms marked with unique identification numbers.

Prohibits pharmacists from dispensing drugs based on written prescriptions not issued on official Ohio forms, with exceptions for electronic/oral orders and institutional facility prescriptions where the patient never possesses the form.

Establishes an Ohio Official Prescription Program Advisory Committee to advise the Board on form compliance, vendor contracts, program evaluation, and impacts on pharmacies.

Requires prescribers to immediately notify the Board of any loss, theft, destruction, or unauthorized use of official prescription forms, and allows the Board to notify pharmacies of such incidents.
