Defines "prosthesis" as an artificial leg, arm, or eye, including replacements required due to changes in the patient's physical condition.

Requires health insuring corporations to cover medically necessary prostheses at least equal to Medicare coverage levels under policies delivered, issued, or renewed in Ohio.

Requires sickness and accident insurers and public employee benefit plans to provide prosthesis coverage meeting Medicare standards for policies issued or plans established/modified after the effective date.

Allows insurers to require prior authorization using standard processes, impose cost-sharing (deductible, copayment, coinsurance) consistent with other covered benefits, and set reimbursement at Medicare fee schedule amounts.

Prohibits annual or lifetime dollar maximums specific to prostheses, except for aggregate maximums applying to all covered services; exempts supplemental policies and specific disease/accident coverage from requirements.
