Department of Job and Family Services shall implement a disease management component within the Medicaid program, excluding recipients already in the care management system under section 5111.16 of the Revised Code.

Disease management component consists of coordinated health care interventions and patient communications for Medicaid recipients with medical conditions where patient self-care efforts are significant.

Component must support physicians and the patient-provider relationship, emphasize prevention of exacerbations using evidence-based guidelines and patient empowerment, and evaluate clinical, humanistic, and economic outcomes on an ongoing basis.

Department may contract with other state agencies under section 5111.91 of the Revised Code to include the disease management component in their administered Medicaid programs.
