Department of Job and Family Services must conduct a review of case management services in the fee-for-service Medicaid component to identify groups not receiving case management and those receiving duplicate services from multiple providers.

Department must implement a new case management component modeled on the former enhanced care management program under section 5111.16 of the Revised Code, with adjustments as necessary for the populations served.

New case management component shall serve Medicaid recipients with physician-diagnosed conditions including high-risk pregnancy, diabetes, asthma, lung disease, congestive heart failure, coronary artery disease, hypertension, hyperlipidemia, HIV infection, AIDS, and chronic obstructive pulmonary disease.

Component applies to Medicaid recipients not enrolled in the existing care management system or Medicaid waiver programs who have one of the specified medical conditions.
