Create the breast and cervical cancer project income tax contribution fund in the state treasury to receive contributions from taxpayers and the public.

Allow taxpayers claiming income tax refunds to designate any portion of their refund to contribute to the new breast and cervical cancer fund, in addition to existing options for contributing to natural areas, wildlife, or military injury relief funds.

Direct the Director of Health to distribute contributed funds to the national breast and cervical cancer early detection program for screening, diagnostic, and outreach services to uninsured and under-insured women.

Require the tax commissioner to provide space on income tax return forms for taxpayers to indicate donations and print descriptions of fund purposes in return instructions.

Require the Director of Health to report to the General Assembly every odd-numbered year in January on the effectiveness of the contribution system, including amounts contributed over the previous five years and how funds were expended.
