Requires applicants for Children's Health Insurance Program, Medicaid, Ohio Works First, and Supplemental Nutrition Assistance Program to specify names and addresses of their employers and employers of individuals whose income counts toward eligibility on application forms.

Directs the Director of Job and Family Services to complete quarterly reports identifying employers with 50 or more employees who employed benefit recipients, including number of such employees and total state benefit costs by employer.

Makes quarterly reports public records under Ohio law but excludes identifying information about individual employees; reports must be shared with General Assembly members, Director of Development, budget officials, and tax credit authority members.

Requires reports be made available on a searchable website within one year, including information from development, budget, and tax credit officials about how the employer data influences decisions on contracts, awards, subsidies, and incentives.

Amends application procedures for Supplemental Nutrition Assistance Program, Ohio Works First, and Healthy Start Program to comply with employer information reporting requirements.
