Limits political contributions to $500 per election period from principals and key employees of entities applying for or holding licenses to operate video lottery terminal games or casinos in Ohio.

Prohibits campaign committees, political action committees, political parties, and other political entities from accepting contributions exceeding $500 from these gaming industry principals and key employees during primary or general election periods.

Requires gaming license applicants and licensees to file lists of all principals and key employees with the Secretary of State and county boards of elections, with initial filings due within 30 days of application and semi-annual updates on January 1st and July 1st.

Defines "principal" to include officers, directors, beneficial owners, lenders, underwriters, and others determined by the State Lottery Commission or Casino Control Commission.

Establishes civil penalties of three times the amount contributed in excess of the $500 limit for violations by gaming industry principals, key employees, and political recipients.
