Requires the Bureau of Examination and Classification to prepare a written re-entry plan for each inmate at the time of examination, unless contraindicated by the nature of the sentence.

Re-entry plans must assist inmates in re-entry into the community and assess their needs upon release.

Directs the Department of Rehabilitation and Correction to adopt rules establishing standards for preparing re-entry plans.

Requires the bureau to collect social and other information to aid in interpreting examinations and preparing re-entry plans.

Makes re-entry plan records available to the adult parole authority for use in imposing post-release control sanctions, granting parole, and making parole and rehabilitation plans.
