Requires students living in on-campus housing at nonprofit institutions, state institutions, and private career schools of higher education to provide documentation of vaccination against meningococcal meningitis and hepatitis B, effective with the academic year beginning July 1, 2010.

Allows students or parents to obtain a waiver form that exempts the vaccination requirement if signed and submitted, based on religious or medical reasons.

Requires institutions to inform students of the vaccination requirement, provide information available on the department of health website about meningitis and hepatitis B risks and vaccines, and inform students about waiver availability.

Exempts institutions from providing or paying for vaccinations themselves; students are responsible for obtaining vaccines.

Directs the Ohio Board of Regents and State Board of Career Colleges and Schools to adopt rules implementing vaccination compliance reporting procedures by institutions.
