Mandates that individuals applying for or renewing any of these licenses or identification cards must be furnished with a next of kin information form to list at least one contact person's name, address, telephone number, and relationship.
-
Specifies that for applicants under 18 years of age who are not emancipated, the contact person must include the parent, guardian, or custodian.

Directs the form to inform individuals they can return it to any registrar or deputy registrar without fee, mail it to the bureau, or submit information online through the internet.
