Amends Ohio Revised Code section 3704.14 to authorize a decentralized motor vehicle inspection and maintenance program in seven counties where the program currently operates, extending authority through June 30, 2017.

Changes the contract extension period from six months to twelve months beginning July 1, 2011, and extends the subsequent vendor contract period from June 2011-2012 to June 2015-2017.

Requires the decentralized program to utilize existing auto repair facilities as licensed inspection and waiver testing facilities, with BAR-97 certified tailpipe emissions analyzers and on-board diagnostic testing equipment.

Modifies the new vehicle exemption from four years to five years, with a new motor vehicle exempt for five years regardless of title transfer.

Changes the requirement for emissions reduction achievements from "at least the same" to "substantially similar" ozone precursor reductions compared to the current program.
