Establishes the Ohio Cancer Incidence Surveillance System to monitor cancer incidence, conduct epidemiologic studies, and identify causal relationships with occupational, nutritional, environmental, or infectious conditions.

Requires physicians, dentists, hospitals, and diagnostic/treatment providers to report cancer cases to the department of health, with expedited reporting (within 30 days) for patients age 25 or younger.

Mandates the department publish quarterly reports on children's cancer incidence rates and biannual reports on adults' cancer rates by census tract on its website, including maps using color-coding to display cancer types by geographical area.

Establishes confidentiality protections for cancer patient data while allowing release for approved medical research projects that meet specific criteria including researcher credentials verification, adequate safeguards, and written agreements prohibiting patient identification.

Creates procedures for investigating potential cancer clusters when two or more children develop specified cancers in the same county/adjacent areas or when adult cancer rates reach or approach national incidence rates, including early diagnosis programs and water/air quality monitoring.
