Creates the breast and cervical cancer project income tax contribution fund in the state treasury to receive taxpayer refund contributions and direct donations.

Allows taxpayers claiming income tax refunds to voluntarily designate any portion of their refund to contribute to the breast and cervical cancer project fund, joining four existing refund contribution options.

Directs the Director of Health to distribute contributed funds to the Ohio breast and cervical cancer project for screening, diagnostic, and outreach services to uninsured and under-insured women.

Requires priority use of funds for services provided by local health departments, federally qualified health centers, and community health centers, with remaining funds available for other providers.

Establishes reporting requirements for the Director of Health to report annually to the General Assembly in odd-numbered years on fund contributions and expenditures, effective for taxable years beginning January 1, 2012.
