Adds a new violation (division B(40)) for insurance agents who provide, issue, modify, alter, or sign certificates of insurance on non-standard forms not promulgated by recognized insurance organizations or the underwriting insurer.

Clarifies that certificates of insurance are not policies of insurance and cannot amend, extend, or alter the coverage provided by the underlying policy or confer additional rights on certificate holders.

Prohibits certificates of insurance from containing references to contracts other than the referenced insurance contract, and specifies that insurance coverage is subject only to the terms, exclusions, and conditions of the actual policy.

Establishes that certificate holders have a legal right to cancellation or nonrenewal notices only if named in the policy or endorsement, and that notice terms are governed by the policy, not the certificate.

Defines "certificate of insurance" to include documents issued as evidence of property or casualty insurance coverage, excluding policies, binders, endorsements, and automobile identification cards.
