Creates a "Diabetes Awareness" license plate that vehicle owners may apply for through the registrar of motor vehicles.

Requires a $25 contribution per application or renewal, plus a $10 administrative fee, to be deposited into the license plate contribution fund.

Directs all contributions from diabetes awareness plates to the American Diabetes Association for distribution equally among Ohio offices for diabetes awareness programs.

Adds section 4503.495 to the Revised Code establishing the diabetes awareness license plate program alongside existing specialty plate programs.

Amends section 4501.21 to include diabetes awareness plate contributions in the license plate contribution fund that supports various charitable organizations.
