• Requires children enrolled in child day-care centers, type A family day-care homes, and licensed type B family day-care homes to provide proof of immunization within 30 days of enrollment and every 13 months thereafter.

• Mandates immunization against 14 specified diseases: chicken pox, diphtheria, Haemophilus influenzae type b, hepatitis A and B, influenza, measles, mumps, pertussis, pneumococcal disease, poliomyelitis, rotavirus, rubella, and tetanus.

• Provides three exceptions: medical contraindication, parent or guardian declination based on conscience or religious convictions, and age-inappropriateness of the vaccine.

• Requires medical statements documenting immunization status or exceptions; allows parents to decline immunization through a designated component on the medical statement.

• Amends rules for child day-care centers, type A family day-care homes, and type B family day-care homes to incorporate immunization requirements in accordance with the new provisions.
