Creates "Hospice and Homecare" license plates available to owners or lessees of passenger cars, noncommercial motor vehicles, recreational vehicles, or other registrar-approved vehicles.

Requires a $25 contribution per application and renewal, plus a $10 bureau administrative fee, regular license tax, and any applicable motor vehicle taxes or fees.

Directs all contributions to the Center for Community Based Care to fund education about hospice and homecare issues for Ohio residents.

License plates display county identification stickers and identifying words or markings designed by the Center for Community Based Care and approved by the registrar.

Adds section 4503.86 to the Revised Code and amends section 4501.21 to include the new license plate in the license plate contribution fund system.
