Creates a "Juvenile Diabetes Research Foundation" license plate that vehicle owners may apply for and register in Ohio.

Requires a $25 contribution per application and renewal, paid into the license plate contribution fund and distributed to Ohio chapters of the Juvenile Diabetes Research Foundation based on the contributor's county of residence.

Requires a $10 bureau administrative fee paid to the state bureau of motor vehicles fund to cover costs of issuing the specialized plates.

License plates must display county identification stickers and include identifying words or markings jointly designed by all Ohio chapters of the Juvenile Diabetes Research Foundation and approved by the registrar.

Amends section 4501.21 of the Revised Code to add section 4503.86 to the list of license plate contribution sources and establishes the distribution mechanism for collected funds.
