Enacts section 2111.011 of the Revised Code to establish a ward's bill of rights and require distribution of the Ohio Guardianship Guide to guardians.

Requires the probate court clerk to furnish the Ohio Guardianship Guide to guardians either upon appointment or upon first filing of required accounts or reports after the effective date.

Mandates that guardians sign a probate court form acknowledging receipt of the Ohio Guardianship Guide and the ward's bill of rights.

Establishes 19 protected rights for wards including dignity, privacy, control over non-delegated decisions, appropriate services, consideration of personal preferences, safe living conditions, marriage, procreation rights, equal treatment, medical information access, confidentiality, legal representation, and the right to petition for modification or termination of guardianship.

Requires updated versions of the Ohio Guardianship Guide to be distributed to guardians upon appointment or first filing of required documents after the guide is revised.
