Establishes the Ohio Parkinson's Disease and Movement Disorder Registry within the Department of Health to collect information on the incidence of Parkinson's disease and other movement disorders in Ohio.

Requires hospitals and healthcare providers treating patients with Parkinson's disease or movement disorders to submit patient information to the registry through electronic means, with an option to report through alternative registries meeting the same standards.

Provides legal protections for healthcare providers furnishing information to the registry, protecting them from liability and professional disciplinary action absent willful or wanton misconduct.

Creates the Ohio Parkinson's Disease and Movement Disorder Advisory Council, appointed by the governor with senate consent, consisting of a pharmacist, epidemiologist, neurologist, patient, and family member to advise the Department of Health on registry implementation.

Restricts release of protected health information to summary, statistical, or aggregate form only, in compliance with existing privacy laws.
