• Requires healthcare providers to inform parents of immunization risks, benefits, and health risks from diseases when parents decline immunization for religious reasons, with the provider's signature indicating this information was provided.

• Applies immunization requirements to college-preparatory boarding schools and their boards of trustees as if they were school districts.
