Creates the Ohio Cancer Incidence Surveillance System Fund in the state treasury to receive taxpayer contributions and direct donations.

Allows taxpayers to designate all or part of their income tax refund to contribute to the cancer surveillance fund when filing their tax return.

Director of Health shall use fund money to improve cancer reporting completeness and timeliness, including supporting the Ohio cancer incidence surveillance system, training hospitals and reporting sources, employing cancer registrars, and expanding electronic reporting of cancer cases.

Adds the Ohio cancer incidence surveillance system fund to the existing list of funds taxpayers can designate refund contributions to, alongside natural areas, wildlife, military injury relief, Ohio history, breast and cervical cancer, and sick children funds.

Effective for taxable years beginning on or after January 1, 2016.
