Creates a program administered by the Ohio Healthier Buckeye Advisory Council to reduce heroin addiction and death rates through contracts with non-profit intermediaries that implement regional projects.

Intermediaries must select service providers, recruit investors, and coordinate services; contracts include metrics, outcomes, and independent evaluators to measure project effectiveness.

State pays for service costs and administration plus a percentage of future savings determined through per-individual, per-action, or total-outcome basis; intermediaries receive payment only for portions of projects deemed effective by evaluators.

Council must cease entering new contracts by July 1, 2023 (five years after enactment) or when total contracted amounts reach $50 million, whichever occurs first.

Establishes Ohio Social Innovation Fund in state treasury; appropriates $100,000 from General Revenue Fund and $1,000,000 from federal TANF Block Grant for fiscal years 2018 and 2019.
