• Establishes comprehensive procedures for Medical Orders for Life-Sustaining Treatment (MOLST) forms that document patient preferences regarding CPR, medical interventions, nutrition/hydration, and comfort care.

• Creates standardized MOLST form requiring signatures from issuing practitioner (physician, physician assistant, certified nurse practitioner, or clinical nurse specialist) and patient or authorized representative.

• Allows MOLST forms to be issued only for individuals with serious illness or frailty for whom death within one year would not be surprising; forms must be reviewed yearly and expire one year and one day after signing if not reviewed.

• Provides immunity from criminal prosecution, civil liability, and professional disciplinary action for healthcare providers, emergency services personnel, and facilities acting in good faith in accordance with valid MOLST forms or DNR identification.

• Amends existing do-not-resuscitate (DNR) laws and updates provisions in sections governing healthcare decisions, physician assistants, and emergency medical services personnel to align with new MOLST procedures.
