Social security numbers shall not be included on certified copies of death certificates for the first five years after a decedent's death unless specifically requested by an authorized person who presents proof of identity.

Authorized persons who may request a death certificate with the decedent's social security number include the spouse, lineal descendants, funeral directors, estate executors or administrators, county veterans service officers, and law enforcement officials.

Media members, private investigators with a class A or B license, and agents of authorized officials may also request certified copies containing the social security number if they meet specified identification requirements.

Licensed funeral directors may request death certificates containing social security numbers on behalf of other authorized persons, except for law enforcement officials.

After five years from the decedent's death, the restriction on including social security numbers on certified death certificates no longer applies.
