Requires the Ohio Department of Public Safety to establish and maintain a database of persons diagnosed with a communication disability or a disability that can impair communication.

Requires the Opportunities for Ohioans with Disabilities Agency to develop a voluntary verification form that includes the person's name, driver's license or state ID number, vehicle license plate numbers, and a physician's, psychiatrist's, or psychologist's signed certification of the diagnosis.

Allows individuals age 18 and older, parents of minor children, and legal guardians to voluntarily register for inclusion in the database by submitting the completed verification form.

Makes the database available to state and local law enforcement officers through the law enforcement automated data system, but designates database information as non-public records exempt from public inspection.

Permits registered individuals, parents, or guardians to request immediate removal from the database by submitting a removal request on the verification form to the Department of Public Safety.
