Establishes definitions for "additional medical data" (data generated and maintained by health care providers for clinical decision-making that is separate from the medical record) and "requested information" (which may include medical records, additional medical data, or both).

Requires patients, personal representatives, or authorized persons to specify in writing whether they are requesting only medical records, only additional medical data, or both when submitting access requests.

Sets tiered fee schedules for copying medical records and additional medical data: patients pay $2.74 per page for first 10 pages, $0.57 for pages 11-50, and $0.23 for pages 51+; other requestors pay an initial $16.84 fee plus per-page charges.

Authorizes health care providers and medical records companies to contract with patients or insurers for copying fees that differ from statutory amounts.

Maintains existing exemptions from disclosure requirements for records covered by confidentiality provisions in state law, federal regulations, and other statutes.
