Creates a "Padua Franciscan High School" license plate in Ohio with a $30 contribution per registration, to be deposited into the license plate contribution fund.

Directs Padua Franciscan High School in Parma to use 50% of contributions for student tuition assistance and 50% for mental and emotional well-being services including counseling, defensive driving instruction, and education on bullying, dating violence, drug abuse, and suicide prevention.

Requires annual reporting by schools receiving license plate contributions to the Department of Mental Health and Addiction Services and motor vehicle registrar by December 1st, with failure to report resulting in contributions being diverted to the state general revenue fund.

Establishes a $10 administrative fee per registration to compensate the Bureau of Motor Vehicles for processing costs.

Amends section 4501.21 to add the new license plate to the existing license plate contribution fund system and modifies section 4503.772 to include reporting requirements for the new school license plate.
