Requires the Registrar of Motor Vehicles and Deputy Registrars to ask individuals renewing motor vehicle registrations if they want to make a one-dollar voluntary contribution to the Second Chance Trust Fund.

Establishes Section 4501.027 of the Revised Code to require solicitation of voluntary contributions from persons applying for or renewing driver's licenses, identification cards, and commercial driver's licenses in addition to vehicle registration renewals.

Requires registrars to make available informational materials from the Department of Health about organ, tissue, and eye donation to all contributors.

Mandates that all voluntary contributions collected monthly be forwarded to the State Treasurer by the fifth day of the following month for deposit in the Second Chance Trust Fund.

Repeals sections 4506.081, 4507.231, and 4507.501 of the Revised Code, consolidating donation contribution collection procedures into the new Section 4501.027.
