Authorizes local health districts to use credit cards in accordance with prescribed requirements and specifies how health districts may identify themselves.

Requires boards of health to adopt written policies for credit card use within three months (or before first holding an account) addressing authorized users, permitted expenses, acquisition and management procedures, receipt submission, card issuance and cancellation, credit limits, and misuse definitions.

Mandates the health district name appear on all credit cards and related presentation instruments, and requires quarterly reviews of card accounts, active cards, expiration dates, and credit limits.

Establishes that officers and employees are personally liable to reimburse the health fund for expenses without itemized receipts, and that knowingly misusing credit card accounts violates section 2913.21 of the Revised Code.

Requires annual reports to the board of health detailing all rewards received from credit card use and excludes procurement cards, gasoline cards, telephone cards, and cards with merchant category code controls from these requirements.
