Permits parents and guardians to be paid for providing personal care or similar services to children or wards under 19 years old enrolled in Medicaid waiver programs, subject to specific requirements.

Requires parent or guardian to be employed by or under contract with a home health agency that has a Medicaid provider agreement.

Requires parent or guardian to be listed on the department of health's nurse aide registry as currently eligible to work in a long-term care facility.

Limits payment eligibility to services classified as "extraordinary care" under federal 1915(c) waiver guidance and only when the child has been assessed as needing such services to avoid ICF/IID placement.

Amends sections 5123.01, 5166.01, 5166.20, 5166.22, 5166.23, and 5166.30 of the Revised Code to establish definitions and requirements for the program.
